Job Summary
We are seeking a highly skilled Registered Nurse to join our Assertive Community Treatment (ACT) team. As a member of our team, you will provide direct nursing services to patients, including assessments, crisis intervention, medication management, and education.
Responsibilities
- Provide direct nursing services to patients, including assessments, crisis intervention, medication management, and education.
- Work closely with other healthcare professionals to develop and implement individualized care plans that promote recovery and independence.
- Conduct home and community-based assessments to identify patient needs and develop care plans.
- Collaborate with other healthcare providers to coordinate care and services.
Requirements
- Bachelor's degree in nursing and two years of experience or an associate degree in nursing and two years of experience as a registered nurse, one of which must have been in a community health program.
- Licensed as a registered nurse by the SC Board of Nursing.
- One year of experience working with adults diagnosed with serious mental illness and working knowledge of psychiatric medications.
