Key Responsibilities

  • Help prevent illness and disease for at-risk populations by linking them to consistent medical care, screenings, and insurance/financial resource plans.
  • Document patient telephone calls, encounters, and other activities in the designated format and in health records systems.
  • Participate in community-level assessments related to identifying needs, gaps, and priorities for focus populations or overall community health.
  • Collaborate with other departments at Crescent to identify any gaps and provide continuity for improved patient care.

Qualifications

  • A high school diploma is strongly preferred.
  • Bilingual skills in English and Marshallese are required.
  • Certification as a Community Health Worker (CHW) is desirable.

Compensation: The estimated annual salary for this role is $50,000-$70,000 depending on experience and qualifications.
