We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at CenterWell Home Health. As a Home Health LPN, you will provide high-quality nursing care to patients in their homes, promoting independence and well-being.
Key Responsibilities- Provide professional nursing services under the direction of a RN, adhering to state and federal regulations, and CenterWell Home Health policies.
- Develop and implement individualized care plans, focusing on patient-centered care and outcomes.
- Collaborate with healthcare teams to ensure seamless care coordination and communication.
- Document patient progress and outcomes, maintaining accurate and timely records.
- Participate in ongoing education and training to enhance clinical skills and knowledge.
- Graduate of an accredited LPN program or equivalent.
- Current nursing license in the practicing state.
- Valid driver's license, auto insurance, and reliable transportation.
- Current CPR certification.
- Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice.
