Job Summary:

We are seeking a skilled Licensed Practical Nurse to join our team at Nuvance Health. As a LPN, you will provide high-quality nursing care to patients in their homes, working closely with our interdisciplinary team to ensure comprehensive care.

Responsibilities:

  • Provide direct skilled nursing care to patients in their homes, under the supervision of an RN.
  • Assess patient needs and develop individualized care plans to meet those needs.
  • Collaborate with the healthcare team to ensure seamless care transitions and optimal patient outcomes.
  • Document patient information and progress in a timely and accurate manner.
  • Participate in ongoing education and training to stay current on best practices and industry developments.

Requirements:

  • Current LPN licensure in the state of Connecticut.
  • One year of experience in home health care or a related field.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.

Working Conditions:

This position requires frequent travel to patient homes, with occasional evening and weekend hours. The successful candidate will be able to work in a fast-paced environment and adapt to changing circumstances.
