Licensed Practical Nurse

Job Summary: We are seeking a skilled Licensed Practical Nurse to join our team at Nuvance Health. As a Licensed Practical Nurse, you will provide high-quality patient care in a home health setting, working closely with our interdisciplinary team to deliver exceptional patient outcomes.
Key Responsibilities:
- Provide direct skilled nursing care to patients in their homes, under the supervision of a Registered Nurse.
- Assess patient needs and develop individualized care plans to meet those needs.
- Collaborate with the healthcare team to ensure seamless care transitions and optimal patient outcomes.
- Document patient progress and outcomes in a timely and accurate manner.
- Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements:
- Current licensure as a Licensed Practical Nurse in the state of Connecticut.
- Minimum of 1 year of experience in home health care or a related field.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
