Key Responsibilities- Level of Care Assessments: Conduct thorough assessments to determine the level of care required for individuals seeking treatment.
- Evaluations and Assessments: Complete required evaluations and assessments, including screening, intake, clinical assessments, and biopsychosocial histories.
- Client Interviews: Conduct face-to-face, virtual, and/or telephonic interviews with individuals presenting for level of care assessment and/or admission to the appropriate program.
- Documentation: Accurately document assessment/intake records, records of referral, and funding source approvals/authorizations, and diagnosis within the electronic health records (EHR) system.
- Case Management: Document all case management and clinical services required by the Single County Authority (SCA), into the Pennsylvania Web Infrastructure for Treatment Services (PA WITS) system and within the companys electronic health records system.
- Electronic Records: Maintain accurate electronic records on each client and ensure the timely completion of tasks, based on current policy and procedure and the appropriate funding source requirements.
- Assessment Protocols: Maintain proficiency in assessment, intake and level of care protocols, utilizing the ASAM criteria.
- Funding Reviews: Conduct admission funding reviews with relevant payor(s) and assist with completion of medical assistance (MA) applications, as needed.
- Treatment Recommendations: Participate in funding source reviews and make recommendations for treatment and lengths of stay as necessary for clients admitted to any of the Gaudenzia facilities.
- Community Resources: Coordinate with agency and community resources to ensure access to the most accurate and up-to-date interim services programs.
- Follow-up: Maintain accurate information for follow-up in the PA WITS system, along with the companys electronic health records system.
- Relationships: Establish and maintain positive relationships with funding, referral, and regulatory agencies.
- Client Support: Follow up with clients after discharge based on regulatory guidelines and document appropriately in PA WITS and the agencys EHR system.
- Education: Acceptable degrees include (but are not limited to): Social Work, Psychology, Addiction, Counseling, Health/Human Services, Criminal Justice, Education, Health Administration, Physical Therapy, and more.
- Licensure: All applicants must hold a professional license through DOC or be credentialed through PCB at the time of hire OR being working toward licensure or certification with a completion date of no longer than 3 years after date of hire.
