Job Title: Assessment/Intake Specialist

Philadelphia Corporation for Aging (PCA) is seeking a highly skilled Assessment/Intake Specialist to join our team. As a key member of our Long Term Care department, you will play a critical role in assessing the needs of older adults and individuals with disabilities, connecting them to community resources, and ensuring they receive the support they need to live independently.

Job Summary:

The Assessment/Intake Specialist will conduct thorough assessments to determine the level of care required by individuals aged 60 and over, utilizing the Functional Eligibility Determination (FED) tool. You will work closely with Assessment Supervisors, healthcare providers, and community resources to develop personalized care plans that meet the unique needs of each individual.

Responsibilities:
  • Conduct assessments to determine the level of care required by individuals aged 60 and over
  • Utilize the FED tool to assess functional eligibility for Home and Community Based Services (HCBS) or facility placement
  • Develop personalized care plans that meet the unique needs of each individual
  • Collaborate with Assessment Supervisors, healthcare providers, and community resources to ensure seamless care coordination
  • Maintain accurate and up-to-date records of assessments and care plans
  • Participate in ongoing training and professional development to stay current on best practices and regulations
Requirements:
  • Bachelor's degree in behavioral science, human services, social work, or related field
  • Minimum one year of experience in social work, case management, or assessment/intake
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Strong organizational and time management skills
  • Ability to maintain confidentiality and handle sensitive information
