Job Title: Emergency Shelter Case Manager

At San Antonio Metropolitan Ministry, we are seeking a highly skilled and compassionate Emergency Shelter Case Manager to join our team. As a key member of our Emergency Shelter Services department, you will play a vital role in providing case management and crisis intervention services to single adults and couples experiencing homelessness.

Job Summary

The Emergency Shelter Case Manager will assess the needs of guests and arrange, coordinate, and advocate for delivery and access to a wide array of programs and services designed to meet the individual client's needs. This position requires a collaborative, client-driven approach to ensure guests are connected to the services and supports necessary for self-sufficiency.

Key Responsibilities
  1. Complete intake and orientation with new shelter guests.
  2. Conduct assessments needed to enroll guests into the Coordinated Entry System for homeless services.
  3. Connect guests to appropriate resources to meet their immediate needs.
  4. Conduct Bio-Psycho-Social assessments and create individualized service plans.
  5. Conduct weekly case management meetings with shelter guests.
  6. Review service plans regularly and provide guests with necessary information and resources.
  7. Communicate assessments, interventions, and progress through timely record keeping and verbal communication.
  8. Advocate for guests' needs and systemic change when necessary.
  9. Approach challenging guests with understanding and compassion.
  10. Attend team meetings and participate in prioritizing guest needs.
  11. Identify and coordinate access to community services and establish partnerships with other agencies.
  12. Provide crisis intervention and transportation services as needed.
  13. Complete necessary documentation and tracking of activities.
  14. Understand and maintain client confidentiality and record-keeping practices.
Requirements
  • Bachelor's degree in social work or related field.
  • Minimum two years of experience working with individuals experiencing homelessness or vulnerable populations.
  • Understanding of approaches and models aligned with SAMMinistries' programmatic offerings.
  • Experience with Harm Reduction, Motivational Interviewing, and de-escalation techniques.
  • Ability to prioritize multiple job responsibilities and communicate professionally.
Preferred Qualifications
  • Bilingual (English/Spanish) skills.
  • Valid Driver's License.
  • Certification in CPR/First Aid/AED, Trauma Informed Care, Harm Reduction, and Housing First.
Work Environment

This position will function in a residential program environment, requiring regular contact with an at-risk population. Appropriate safety considerations will be required to avoid illness or injury.

Physical Demands

The physical demands of this job include standing, sitting, walking, stooping, talking, hearing, reaching, and using hand and finger dexterity.
