Job SummaryThis is a full-time position with a weekend commitment, working day shifts from 8:00am-4:30pm. You will be responsible for providing professional, comprehensive, family-oriented services to individuals in their homes.
Key Responsibilities- Assess patients' and families' psychosocial, environmental, and financial needs.
- Formulate, implement, and evaluate a plan of care in collaboration with the patient, family, and other caregivers.
- Provide case management as appropriate.
- Offer counseling services to support patients and families during the dying and grieving process.
To be successful in this role, you will need:
- A Master's Degree in Social Work from an accredited school.
- A minimum of three years of clinical experience, with one year in a community health setting preferred.
- An LCSW certification or equivalent.
