About the Role
We are seeking a skilled Hospice Social Worker to join our team at LifeBridge Health, a leading healthcare provider in Maryland. As a Hospice Social Worker, you will play a critical role in delivering compassionate and comprehensive care to patients and their families.
Key Responsibilities:
- Assess patients' and families' psychosocial, environmental, and financial needs
- Formulate, implement, and evaluate a plan of care in collaboration with patient, family, and other caregivers
- Provide counseling services to support patients and families during the dying and grieving process
- C Coordinate with community and agency homecare referrals for services
Requirements
- Masters Degree in Social Work from an accredited school
- Minimum 3 years of clinical experience, with 1 year in a community health setting preferred
- LCSW certification (preferred)
- Professional licensure in Maryland and Pennsylvania
