Primary Care Physician

The Primary Care Physician serves as a medical leader, performing comprehensive assessments and coordinating with a multidisciplinary team to develop and implement care plans for seniors.
Under the direction of the Medical Director, the Primary Care Physician oversees the medical care of participants in nursing homes and hospitals, including admissions, discharges, and the use of medical specialists to ensure effective care coordination.
Key Responsibilities:- Provide medical direction and oversight to Nurse Practitioners and other members of the care team
- Collaborate with community-based physicians and the care team to develop and implement discharge plans from acute or long-term settings
- Participate in the care team, communicating participant changes, collaborating on care planning decisions, and coordinating 24-hour care delivery
- Conduct regular visits to nursing homes to maintain care of participants
- Participate in rotating night and weekend call
- Successful completion of a residency program accredited by the Accreditation Council for Graduate Medical Education
- Doctorate Degree
- Board Certification – Internal Medicine or Family Medicine or Emergency Medicine or Geriatrics (preferred)
- Active California Physician License without Restrictions
- Reliable means of transportation
- Active state CDS Certification and DEA
- Must be currently enrolled or eligible for enrollment to provide services to Medicare and Medi-Cal participants

Salary/Wage base range for this role is $250,000 - $280,000 / year + Bonus + Equity. WelbeHealth offers a competitive total rewards package that includes, 401k match, healthcare coverage and a broad range of other benefits.
Actual pay will be adjusted based on experience and other qualifications.
