Primary Care Physician

WelbeHealth is seeking a highly skilled Primary Care Physician to join our team. As a Primary Care Physician, you will be responsible for providing comprehensive medical care to our participants, including assessment, planning, and coordination of care.
Key Responsibilities:- Provide assessment, planning, and primary care to participants
- Collaborate with community-based physicians and interdisciplinary teams to ensure effective care coordination
- Actively participate in interdisciplinary team meetings to communicate participant changes and collaborate on care planning decisions
- Help maintain care of participants in nursing homes, making regular visits as dictated by participant need
- Participate in rotating night and weekend call
- Successful completion of a residency program accredited by the Accreditation Council for Graduate Medical Education
- Doctorate Degree
- Board Certification – Internal Medicine or Family Medicine or Emergency Medicine or Geriatrics (preferred)
- Active California Physician License without Restrictions
- Reliable means of transportation
- Active state CDS Certification and DEA
- Must be currently enrolled or eligible for enrollment to provide services to Medicare and Medi-Cal participants

Salary/Wage base range for this role is $250,000 - $280,000 / year + Bonus + Equity. WelbeHealth offers competitive total rewards package that includes, 401k match, healthcare coverage and a broad range of other benefits. Actual pay will be adjusted based on experience and other qualifications.
