About the Role

We are seeking a skilled Physical Therapist Assistant to join our innovative Delta Health Rehab at Home program. As a PTA, you will play a vital role in delivering high-quality outpatient physical therapy services directly to patients' homes.

Key Responsibilities
  • Administer physical therapy to patients in their place of residence, in accordance with physician orders and plan of care under the direction and supervision of the Physical Therapist (PT).
  • Provide physician-prescribed physical therapy under a plan of care established by the PT.
  • Improve or minimize residual physical disabilities of the patient.
  • Return the individual to optimum and productive level of function within the patient's capabilities.
  • Participate with all other healthcare personnel in patient care planning.
  • Perform all skilled procedures as ordered by physician and according to the plan of care established by the PT.
  • Consult with PT regarding significant change in patient status and/or treatment plan.
  • Complete patient and/or family member education, including in-home exercise programs, activities of daily living, and safe mobility practices.
  • Participate in performance improvement activities as assigned.
  • Attend all patient care conferences as scheduled.
  • Review medical record, plan of care, and goals for each patient visit, and complete thorough and concise documentation for each treatment in a timely manner as per Hospital policy.
Requirements
  • Graduate from a two-year college-level program approved by the American Physical Therapy Association.
  • Currently licensed in the state(s) in which practicing.
  • Three (3) years' experience, preferred.
  • Background in Home Health and Outpatient settings, preferred.
  • Acceptance of philosophy and goals of Hospital.
  • Ability to exercise initiative and independent judgment.
