We are seeking a skilled and compassionate MDS Coordinator to join our healthcare team at Avamere Living. As a key member of our nursing staff, you will play a vital role in developing and implementing patient care plans that meet the unique needs of our residents.

Key Responsibilities:
  • Develop and maintain accurate and comprehensive patient care plans in collaboration with the interdisciplinary team.
  • Conduct regular reviews of patient progress and make recommendations for adjustments to care plans as needed.
  • Collaborate with nursing staff to ensure seamless delivery of patient care services.
  • Participate in quality improvement initiatives to enhance the overall quality of patient care.
  • Provide education and training to nursing staff on best practices and new procedures.
Requirements:
  • Current, unencumbered RN license in the state of Oregon.
  • 1+ years of experience as a supervisor in a healthcare setting.
  • Training in rehabilitative and restorative nursing practices.
  • Knowledge of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to nursing care communities.
  • Experience with electronic medical records and computer documentation systems.
