Job Summary

We are seeking a skilled and compassionate RN MDS Coordinator to join our team at Avamere Living. As a key member of our nursing department, you will play a vital role in developing and implementing care plans that meet the unique needs of our residents.

Key Responsibilities
  • Develop and implement care plans that align with resident goals and preferences
  • Collaborate with interdisciplinary teams to ensure seamless care delivery
  • Conduct regular rounds to monitor resident care and address concerns
  • Provide education and training to nursing staff on best practices and new procedures
  • Participate in quality improvement initiatives to enhance resident outcomes
Requirements
  • Current RN licensure in the state of Oregon
  • 1+ year of experience in a supervisory role in a healthcare setting
  • Training in rehabilitative and restorative nursing practices
  • Knowledge of electronic medical records and computer documentation systems
  • Passion for caregiving and serving our senior resident community
