Job Summary

Aveanna is seeking a skilled Registered Nurse to provide high-quality care to patients in their homes. As a Home Health RN, you will be responsible for developing and implementing individualized care plans, educating patients and their families, and coordinating with healthcare teams to ensure seamless care.

Key Responsibilities:
  • Develop and implement care plans through physician orders, client input, and nursing assessment
  • Educate clients and their family members on specific needs and care requirements
  • Properly orient and train primary caregivers to ensure optimal functioning levels
  • Coordinate continuum of care through documentation and timely communication with physicians and caregivers
  • Provide skilled nursing care in patients' homes, including phone triage, patient education, observation/assessment, wound care, infusions, and more
Requirements:
  • Active RN License in the state of application
  • Preferred: Medicare Skilled Nursing experience, basic understanding of Oasis, and 1-year RN experience in a healthcare setting
