Job Summary

We are seeking a skilled and experienced LCSW to join our dedicated team. The ideal candidate will have a DC License in hand and possess excellent social work and discharge planning skills.

Key Responsibilities:
  • Provide exceptional patient care and support to patients and their families
  • Develop and implement effective discharge plans to ensure smooth transitions for patients
  • Collaborate with healthcare teams to coordinate care and services
  • Manage and coordinate patient referrals, admissions, and authorizations
  • Ensure compliance with regulatory requirements and industry standards
Requirements:
  • DC License in hand
  • Experience with discharge planning and social work
  • Excellent communication and interpersonal skills
  • Ability to work in a fast-paced environment and prioritize tasks effectively
  • Certifications in BLS, RN, or LMSW/LGSW/LCSW
Preferred Qualifications:
  • Experience with Cerner and other healthcare software systems
  • Knowledge of regulatory requirements and industry standards
  • Ability to work in a collaborative environment and prioritize tasks effectively
