Job Title: Wraparound Family Partner

We are seeking a dedicated and compassionate individual to join our team as a Wraparound Family Partner. In this role, you will provide support to individuals with mental health or substance abuse issues in various settings, including clinics, residential, school, workplace, and community environments.

Job Responsibilities:
  • Develop and implement Person-Centered Plans in collaboration with the interdisciplinary healthcare team.
  • Foster relationships and serve as a liaison between the company, parents, guardians, local agencies, and the community.
  • Provide supportive counseling to minimize the negative effects of psychiatric symptoms or substance dependence.
  • Support clients in developing daily and community living skills, socialization skills, adaptation skills, and behavior and anger management.
  • Perform case management functions, including linking and arranging for services and referrals.
  • Collaborate with other clinical/professional staff to maintain communication and provide feedback, standardizing procedures and expediting PCP implementation.
  • Monitor utilization of services to ensure effectiveness, appropriateness, and adherence to rules, PCP, and service authorization.
Requirements:
  • A GED or high school diploma is required.
  • A minimum of one year of documented experience working with the population to be served or providing the type of service is preferred.
  • A valid driver's license and automotive insurance are required.
