We are seeking a skilled and compassionate Home Health Physical Therapist to join our team at AccentCare. As a leading provider of post-acute healthcare services, we are committed to delivering high-quality, patient-centered care that makes a meaningful difference in the lives of our patients.

About the Role

This is a full-time position offering a competitive salary range of $90,000-$105,000 per year, paid on a per-visit basis. The compensation reflected is an estimate of annual compensation based on the location and duration of the assignment. We also offer a $20,000 bonus for eligible candidates.

Key Responsibilities
  • Deliver high-quality physical therapy services to patients in their homes, focusing on improving mobility, reducing pain, and enhancing overall well-being.
  • Conduct thorough assessments, develop personalized treatment plans, and provide evidence-based interventions to achieve optimal outcomes.
  • Collaborate with interdisciplinary teams, including physicians, nurses, and other healthcare professionals, to ensure seamless coordination of care.
  • Communicate effectively with patients, families, and caregivers to educate them on treatment goals, progress, and home exercise programs.
  • Maintain accurate records, document patient interactions, and participate in quality improvement initiatives to enhance patient satisfaction and clinical outcomes.
Requirements
  • Minimum nine months experience as a licensed physical therapist in a community or home health setting.
  • Preferred qualifications include relevant certifications, such as OCS (Orthopedic Certified Specialist) or CHT (Certified Hand Therapist).
  • Possession of a Master's degree in physical therapy from an accredited program, current licensure to practice as a physical therapist within the state of operation, and valid CPR certification.
  • Able to travel to all business locations and possess a valid driver's license.
