Key Responsibilities- Coordinate and administer nursing care for patients of all ages, including acute, chronic, and preventative services.
- Work with patients' providers to establish goals and meet identified health care needs.
- Develop individual guidelines for patient-specific education and provide educational resources to patients and families.
- Provide triage services by phone and in-person, using approved algorithms and health assessment guidelines.
- Assist with or institute emergency measures as needed, including the use of emergency equipment.
- Review and reconcile medication lists, alerting providers to any discrepancies.
- Participate in team conferences and development of nursing care plans.
- Perform various lab studies and procedures under the direction of a provider.
- Document all data collected and procedures performed in patients' electronic medical records.
- Vermont licensed RN with 1-2 years of clinical experience in an office setting.
- Understanding of medical and social aspects of chronic and acute disease affecting patients.
- Computer literacy and ability to learn and comprehend electronic medical record systems.
- Ability to work in a busy office and maintain a cooperative and professional attitude.
- Ability to greet and interact with the public in a tactful and compassionate manner.
This position requires manual dexterity, good or corrected vision and hearing, and the ability to move up to 50 pounds at one time. You may be exposed to infectious and contagious diseases, and regular exposure to the risk of blood-borne diseases.
