We are seeking a skilled RN Care Coordinator to join our team at Riverside Health System. As a RN Care Coordinator, you will play a critical role in coordinating patient care and services across the continuum of care, promoting effective utilization and monitoring of healthcare resources.
Key Responsibilities:
- Collaborate with the healthcare team to coordinate patient care and services
- Assess patient needs and develop plans of care to ensure timely and effective transitions
- Communicate with payors and community resources to ensure seamless care transitions
- Document all assessments and discharge planning changes in the EMR
- Participate in daily interdisciplinary planning meetings and weekly outlier case reviews
Requirements:
- Program Graduate, Nursing - Diploma Program (Required) or Associates Degree, Nursing (Required) or Bachelors Degree, Nursing (Preferred)
- 3-4 years Clinical experience required (may include LPN experience) (Required)
- 1 year Case management experience (Preferred)
- Registered Nurse (RN) - Virginia Department of Health Professions (VDHP) Upon Hire (Required)
- Accredited Case Manager (ACM) - American Case Management Association (ACMA)/CCM Upon Hire (Preferred)
