GENERAL SUMMARY:
The Document Imaging Chart Recovery Specialist plays a crucial role in the healthcare documentation process. This position is responsible for the efficient collection and conversion of discharged medical records into Electronic Medical Records (EMR), ensuring that healthcare professionals have timely access to vital patient information for effective treatment and care.
KEY RESPONSIBILITIES:
- Collect discharged medical records from various units and departments within the facility.
- Maintain accurate logs of received records and prepare them for transport to the centralized imaging office as per facility protocols.
- Process and route loose reports or transferred charts promptly.
- Conduct concurrent scanning of nursing units and discharges following established protocols.
- Ensure the availability of necessary supplies for scanning and validation tasks.
- Manage the timely processing of loose report batches.
- Oversee the maintenance of document imaging equipment and supplies, alerting leadership when inventory is low.
- Complete all required documentation for chart tracking accurately.
- Verify that all pages have appropriate patient identification and barcodes.
- Review paper records against electronic documents to ensure image quality and completeness.
- Identify and rectify image/document errors as necessary.
- Accurately direct paper documents for scanning into the correct medical records locations.
- Monitor the Cer-Batch Index application to confirm that charts have been properly processed and posted for access.
- Follow up on missing charts and notify relevant leadership and imaging supervisors.
- Maintain records in temporary storage until the designated destruction date, adhering to state and corporate policies.
- Meet defined schedules and expectations for task completion.
- Maintain productivity and quality standards as per current guidelines.
- Perform additional duties as assigned.
REQUIRED KNOWLEDGE AND SKILLS:
- Intermediate to advanced proficiency in computer applications, including Microsoft Office Suite.
- Strong troubleshooting, analytical, and comprehension skills.
- Excellent organizational, communication, and multitasking abilities.
- Understanding of security and confidentiality regulations regarding protected health information.
- Effective communication skills.
- Prior experience in document imaging within a healthcare setting is essential.
- High school diploma or equivalent is required.
PREFERRED KNOWLEDGE AND SKILLS:
- Familiarity with medical terminology.
- Previous experience in document imaging and clerical tasks.
