About the Role

Innovative Integrative Health is seeking a skilled Licensed Vocational Nurse - Home Care Specialist to join our team. As a key member of our healthcare team, you will provide high-quality patient care services to support our Primary Care Clinic, Adult Day Health Center, and Home Care programs.

Key Responsibilities
  • Perform patient care services under the clinical supervision of the Primary Care Provider (PCP), Nurse Practitioner (NP), or other licensed staff.
  • Maintain a clean, organized, and safe environment.
  • Participate in IDT and Care Plan Meetings as scheduled.
  • Assist in prepping patients for assessments or procedures during clinic or home visits.
  • Complete comprehensive assessments of participants during pre-enrollment encounters, semi-annual, unscheduled, and annual assessments.
  • Perform and complete Home Care assessments as scheduled per policy guidelines.
  • Collaborate with Home Care Coordinator - Licensed Vocational Nurse Lead (HCC LVN Lead) and Personal Care Assistant Supervisor to ensure participants receive allocated time and services based on assessments.
  • Involved in the development and implementation of Home Care Planning activities.
  • Document patient concerns, change of condition (COC), and any changes in participants' baseline status to appropriate staff daily.
  • Review, follow up, and document any follow-ups and Service Delivery Requests in a timely manner per policy guidelines.
  • Provide support to participant needs with ADLs and iADLs as needed.
  • Enter and concurrently acknowledge pertinent patient information, results, and collected data into electronic health record system.
  • Ensure patients and their families understand discharge or after-visit instructions.
  • Ensure proper body mechanics and patients' safety with ambulating/transfer, etc.
  • Attend and participate in staff meetings, in-services, projects, and committees as assigned.
  • Adhere to and support the center's practices, procedures, and policies, including assigned break times and attendance.
  • Accept assigned duties in a cooperative manner and perform all other related duties as assigned.
Requirements
  • Minimum of one (1) year of demonstrated successful experience in the Quality healthcare field of work.
  • Minimum of one (1) year of documented experience working with a frail or elderly population.
  • Active LVN license in the state of California.
  • IV certification preferred.
  • CPR certification.
  • Is medically cleared for communicable diseases and has all immunizations up-to-date before engaging in direct participant contact.

  • Ability to lift up to 35 pounds occasionally, 15 pounds frequently, and 7 pounds constantly; required to obtain assistance of another qualified employee when attempting to lift or transfer objects over 25 pounds.
  • Requires constant hand grasp and finger dexterity; frequent sitting, standing, walking, and repetitive leg and arm movements, occasional bending, reaching forward and overhead; squatting and kneeling.
  • Ability to communicate verbally with an excellent comprehension of the English language.
