Key Responsibilities:

As a Licensed Vocational Nurse, you will deliver in-home care as part of a proactive, interdisciplinary strategy for managing patients with complex, chronic, and palliative conditions. This role is perfect for a dedicated clinician eager to influence the future of healthcare delivery in a fast-paced setting. Your specific duties will include:
  • Evaluating patients' health status during each visit and documenting findings in the electronic medical record.
  • Conducting assessments, including vital signs, medical history intake, and medication reconciliation, while communicating results to the healthcare provider.
  • Supporting the outreach team in engaging patients within the community.
  • Coordinating with various healthcare professionals, including Occupational and Physical Therapists, Physicians, and others involved in the patient's care.
  • Providing palliative care to ensure patient comfort.
  • Recommending tools or devices that may enhance the patient's quality of life.
  • Listening attentively to patients and addressing their concerns or requests.
  • Traveling to multiple patient homes as needed.
  • Performing blood draws and preparing specimens for laboratory analysis.
  • Educating patients on general health and self-care practices.
  • Clarifying physician orders with patients.
  • Instructing patients on the use of home medical equipment.
  • Participating in bi-weekly clinical rounds for panel management.
  • Delivering care in various settings, including shelters, transitional housing, assisted living facilities, and patients' homes.
  • Receiving guidance from registered nurses, nurse practitioners, or physicians via remote connections or telephone.
  • Assisting with outreach efforts to locate and enroll hard-to-reach patients.
  • Carrying out additional duties as assigned.
Qualifications:
  • Minimum of one year of experience in an outpatient or home healthcare environment.
  • Strong communication and time management abilities.
  • Current California LVN license and certification from an accredited nursing program.
  • Ideal candidates will possess excellent clinical skills, be technology proficient, certified in phlebotomy, CPR certified, and demonstrate strong customer service skills.
