Job Requirements:

  • Graduate of an accredited school of nursing with a current state license as a licensed practical nurse
  • 2+ years of nursing experience in an acute care setting, or equivalent experience (hospice experience preferred)

Responsibilities:

  • Conduct patient visits and deliver routine care
  • Communicate effectively with the care team and families
  • Work under the supervision of a registered nurse
  • Provide compassionate care and support to patients and their families
