Job Summary

We're seeking a highly skilled Care Management Specialist-ECM to join our team. In this role, you'll support enhanced care management patients with behavioral health conditions, severe mental illnesses, and substance use disorders needs using brief interventions and behavioral activation strategies.

Key Responsibilities
  • Utilize evidence-based brief behavioral health therapeutic interventions to improve patients' ability to manage their own health
  • Provide care coordination and connection to services and social supports for ECM patients, including appointment scheduling and referral management
  • Educate patients and families about chronic medical and behavioral health conditions to improve health literacy
  • Assist with the coordination of medical and behavioral health access issues to primary care physician (PCP) offices, specialists, and ancillary services
  • Track medical and behavioral health outcome measures in a patient registry
  • Coordinate with psychiatric hospitals to ensure smooth transitions of care
  • Participate in weekly systematic and ad hoc case reviews to collaborate with ECM team on patient care issues
  • Work with patients to identify health and wellness goals to incorporate into patients shared care plan with providers
  • Review patient community health assessments (CHAs) upon completion by another care team member to ensure completed accurately
  • Maintain engagement with patients in a manner that utilizes evidence-based approaches that promotes collaboration between the patient and their medical/behavioral team
  • Document care coordination, education, and other services provided in eClinicalWorks
  • Provide accurate and timely caseload status reports and updates to supervisor and site staff
Requirements
  • High school diploma/GED required; bachelor's degree in healthcare/social sciences related field preferred
  • Four years of experience in healthcare required
  • Experience with community outreach, behavioral health, and working with high-risk populations preferred
  • Bilingual (English/Spanish) preferred
