About the Role

We are seeking a skilled and compassionate Registered Nurse to join our Home Health team. As a Home Health Registered Nurse, you will play a vital role in providing high-quality patient care and support to individuals in their homes.

Key Responsibilities
  • Provide admission, case management, and follow-up skilled nursing visits for home health patients.
  • Confer with physicians to develop and revise patient care plans based on ongoing assessments and medical orders.
  • Implement patient care plans in conjunction with patients and families to achieve optimal resolution of needs and problems.
  • Coordinate and oversee the work of Home Health Aides, Certified Home Health Aides, and Personal Care Workers.
  • Monitor and document patient care and progress, including case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes.
Requirements
  • Diploma, Associate, or Bachelor's Degree in Nursing.
  • Minimum of one year of nursing experience, preferably in med-surg, ICU, ER, or acute care settings.
  • Current and unrestricted Registered Nurse licensure.
  • Current CPR certification.
  • Strong organizational and communication skills.
  • A valid driver's license, auto insurance, and reliable transportation.
