POSITION SUMMARY

The primary responsibility of this role is to deliver direct nursing care to residents while overseeing the daily nursing operations conducted by nursing aides. This oversight must align with existing federal, state, and local standards, guidelines, and regulations governing our facility, as directed by the Director of Nursing Services or Nurse Supervisor, to ensure the highest quality of care is consistently upheld.

AUTHORITY AND RESPONSIBILITY:

As a Practical Nursing Specialist, you are entrusted with the administrative authority, responsibility, and accountability essential for executing your designated duties in compliance with state regulations.

KEY FUNCTIONS AND DUTIES:

ADMINISTRATIVE DUTIES
  • Guide the daily operations of nursing aides in accordance with current regulations and guidelines governing long-term care facilities.
  • Ensure compliance of all nursing staff with the facility's established policies and procedures.
  • Review and recommend updates to departmental policies, procedure manuals, and job descriptions.
  • Collaborate with nursing staff and support personnel to plan services, programs, and activities for each shift.
  • Maintain the Nursing Service Procedures Manual to reflect current nursing practices.
  • Ensure adherence to procedures outlined in the Nursing Service Procedures Manual by all nursing personnel.
  • Prepare written and oral reports regarding shift activities as required.
  • Coordinate with other resident services to maintain a comprehensive regimen of care.
  • Participate in the development and implementation of the facility's quality assurance program for nursing services.
  • Engage in facility surveys conducted by authorized government agencies as requested.
  • Review and update resident discharge plans as necessary.
  • Assist in planning nursing services for resident discharge as needed.
  • Interpret departmental policies and procedures to staff, residents, visitors, and regulatory agencies as required.
  • Manage the admission, transfer, and discharge processes for residents.
  • Complete necessary incident reports as required.
  • Maintain the Daily Census Report and submit it as needed.
  • Perform administrative tasks such as completing medical forms, reports, evaluations, and charting as necessary.
DOCUMENTATION AND RECORDKEEPING
  • Complete and file necessary recordkeeping forms upon resident admission, transfer, and discharge, ensuring compliance with the center's medical records system.
  • Encourage attending physicians to review and sign treatment plans and progress notes as per established policies.
  • Receive and document telephone orders from physicians accurately.
  • Transcribe physician orders to resident charts and treatment plans as required.
  • Document nursing notes in a clear and descriptive manner reflecting the care provided and the resident's response.
  • Complete incident reports and submit them to the Director as required.
  • Record any discrepancies noted regarding physician orders or charting errors and report them to the Nurse Supervisor.
  • Ensure proper documentation for unauthorized discharges is entered in the resident's medical record.
  • Perform routine charting duties in accordance with established policies.
  • Sign and date all entries made in the resident's medical record.
MEDICATION ADMINISTRATION
  • Prepare and administer medications as prescribed by the physician.
  • Verify resident identity prior to administering medications or treatments.
  • Ensure medications prescribed for one resident are not given to another.
  • Maintain an adequate supply of medications and report needs to the Nurse Supervisor.
  • Order prescribed medications and supplies as necessary.
  • Ensure accuracy in narcotic records for your shift.
  • Notify the Nurse Supervisor of any discrepancies in drug and narcotic records.
  • Review medication cards for completeness and accuracy.
  • Notify physicians of automatic stop orders prior to the last dosage being administered.
  • Dispose of medications as required by established procedures.
PERSONNEL MANAGEMENT
  • Participate in employee performance evaluations and staffing recommendations.
  • Inform the Nurse Supervisor of staffing needs when personnel are absent.
  • Report absenteeism to the Nurse Supervisor.
  • Review departmental workforce and make staffing recommendations.
  • Develop work assignments and assist in their completion.
  • Provide leadership to nursing personnel on your unit.
  • Conduct daily rounds to ensure nursing staff are fulfilling their responsibilities.
  • Ensure nurse aide trainees are supervised by licensed nurses.
  • Meet regularly with nursing personnel to address issues and improve services.
  • Foster a collaborative environment with inter-departmental personnel.
  • Maintain a warm and positive atmosphere within the unit.
  • Address complaints and grievances from staff and report them to the Nurse Supervisor.
  • Ensure fair disciplinary actions are taken without bias.
  • Exchange nursing reports at the beginning and end of shifts.
  • Report occupational exposures to blood and hazardous materials according to facility policies.
NURSING CARE RESPONSIBILITIES
  • Inform nursing staff of new admissions and their expected arrival.
  • Ensure readiness of rooms for new residents.
  • Participate in orienting new residents and their families to the facility.
  • Accompany physicians during rounds as necessary.
  • Arrange for diagnostic and therapeutic services as ordered by physicians.
  • Consult with physicians regarding resident care and treatment.
  • Review resident charts for treatment and medication orders.
  • Implement established nursing objectives and standards in accordance with care plans.
  • Monitor certified nursing assistants to ensure proper treatment administration.
  • Coordinate social and activity programs with nursing schedules.
  • Notify physicians and family members of changes in resident conditions.
  • Carry out restorative and rehabilitative programs as needed.
  • Inspect nursing service areas daily for cleanliness and safety.
  • Administer professional services as required within the scope of practice.
  • Use restraints when necessary, following established policies.
  • Obtain specimens for lab tests as ordered.
  • Record vital signs and monitor residents as necessary.
  • Check food brought in by family for dietary compliance.
  • Ensure direct care is in accordance with resident care plans.
  • Conduct frequent checks on residents unable to call for help.
  • Meet with residents and families as necessary to address concerns.
  • Assist in arranging transportation for discharged residents.
  • Ensure discharged residents are escorted appropriately.
  • Notify family members of resident deaths and follow post-mortem procedures.
STAFF DEVELOPMENT
  • Participate in developing and conducting in-service training for nursing staff.
  • Implement an effective orientation program for new employees.
  • Assist in standardizing work methods and procedures.
  • Train staff on tasks involving potential exposure to blood and body fluids.
  • Attend continuing education programs to stay updated in the field.
SAFETY AND SANITATION
  • Monitor staff adherence to safety regulations and proper equipment use.
  • Ensure compliance with departmental policies, including dress codes.
  • Assist in identifying tasks with potential exposure to blood and body fluids.
  • Ensure adequate supplies of personal protective equipment are available.
  • Maintain cleanliness in work areas and resident care rooms.
  • Ensure proper handwashing techniques are followed by staff.
  • Participate in fire safety and disaster preparedness drills.
  • Assist in developing infection control programs.
  • Ensure compliance with established procedures for hazardous conditions.
  • Report any missing or illegible safety labels and MSDSs.
EQUIPMENT AND SUPPLIES
  • Recommend equipment and supply needs to the Nurse Supervisor.
  • Maintain adequate stock levels of medications and supplies.
  • Participate in safe operation procedures for nursing equipment.
  • Monitor nursing procedures to prevent waste of supplies.
CARE PLAN FUNCTIONS
  • Review care plans to ensure adherence to established protocols.
  • Ensure nursing notes reflect compliance with care plans.
  • Keep certified nursing assistants informed of resident care plans.
  • Assist in planning and revising resident assessments and care plans.
RESIDENT RIGHTS
  • Maintain confidentiality of resident care information.
  • Ensure residents are treated with dignity and respect.
  • Provide nursing care in privacy and respect residents' rights.
  • Address and document resident complaints and grievances.
  • Investigate allegations of resident abuse and report findings.
  • Honor residents' requests to refuse treatment as per facility policies.
ADDITIONAL RESPONSIBILITIES
  • Provide data to the Quality Assurance & Assessment Committee as needed.
WORKING ENVIRONMENT
  • Work in various areas of the nursing service, including offices and resident rooms.
  • Move intermittently throughout the workday and manage frequent interruptions.
  • Engage with residents, families, and personnel under various conditions.
  • Be prepared for emergency situations requiring immediate response.
  • Participate in continuing education programs to enhance skills.
  • Manage exposure to potential hazards and infectious materials.
EDUCATION AND EXPERIENCE
  • Must be a graduate of an accredited LPN/LVN program.
  • Participation in continuing education is encouraged.
QUALIFICATIONS
  • Must hold a current, unencumbered license to practice as an LPN/LVN.
  • Ability to communicate effectively in English, both verbally and in writing.
  • Ability to make independent decisions when necessary.
  • Ability to interact tactfully with residents, families, and staff.
  • Knowledge of nursing practices and regulations pertaining to long-term care.
  • Ability to develop and implement quality care programs.
  • Must possess patience, tact, and a positive attitude.
  • Willingness to learn and adapt to new nursing methods.
  • Ability to report on resident conditions accurately.
  • Must not pose a direct threat to the health or safety of others.
PHYSICAL AND SENSORY REQUIREMENTS
  • Must be able to move throughout the facility during working hours.
  • Must communicate effectively in English.
  • Must cope with the mental and emotional demands of the position.
  • Must possess adequate sight and hearing or use prosthetics as needed.
  • Must work independently and effectively with residents and staff.
  • Must meet health requirements set by facility policies.
