Job Summary:
The Psychiatric Rehabilitation Services Coordinator provides group and individual therapy, including psychiatric rehab and case management services to adults with a history of psychiatric hospitalizations. This role involves helping patients with long-term care and/or stabilization, and being part of an interdisciplinary team in developing and implementing therapeutic services and interventions to best serve their needs.
Key Responsibilities:
- Provide group and individual therapy, including psychiatric rehab and case management services
- Help patients with long-term care and/or stabilization
- Participate in an interdisciplinary team to develop and implement therapeutic services and interventions
- Work with patients to help them achieve their goals and improve their quality of life
Requirements:
- Prior experience as a PRSC or mental health professional in a licensed nursing home, intermediate care facility, or specialized mental health rehab facility
- Bachelor's Degree in a human services field, including sociology, special education, rehabilitation counseling, or psychology
- Minimum of 1 year of supervised experience in mental health or human services setting
- Experience with Point Click Care (PCC) is ideal, but not required
