General Overview of the Role

The Clinical Care Coordinator is tasked with the seamless management of patient care throughout the healthcare continuum, adhering to the prescribed treatment plans set forth by healthcare providers, national standards, and nursing regulations.

This role involves educating and supporting patients to enhance their understanding of the treatment protocols established by licensed healthcare professionals, ensuring that care is holistic, culturally sensitive, and evidence-based.

The Clinical Care Coordinator plays a pivotal role in monitoring patient outcomes and engaging in quality enhancement initiatives, working collaboratively with healthcare team members to improve patient results and experiences.

This position is recognized as a professional exemplar and clinical authority, fostering an environment that promotes nursing excellence and collaborative decision-making.

Key Responsibilities

Assessment:
Collects comprehensive information to formulate personalized care plans.

Planning:
a.

Collaborates with an interdisciplinary team to create and oversee the patient's care plan.

b. Aids patients and families in scheduling necessary tests and services, providing essential medical history and diagnostic information.
c. Assists at-risk patients and their families with risk evaluations, screenings, and educational resources by liaising with relevant services.
d.

Coordinates ongoing care, including community services, outreach programs, home care, palliative, and hospice services as required.


Patient Education:
a.

Delivers initial and ongoing education regarding specific health conditions, treatment options, and the agreed-upon care plan for patients and their families.

b. Serves as a resource for community education and outreach initiatives.

Implementation:
a.

Acts as a liaison among patients, families, healthcare teams, community resources, and other facilities to ensure coordinated care delivery.

b. Advocates for patients to identify and remove barriers to care.
c. Streamlines the referral process to enhance the efficiency of care delivery.
d.

Ensures timely and effective transitions into specialized services, anticipating the needs of patients and families throughout their care journey.

e.

Connects patients with appropriate resources, healthcare services, and support systems within MedStar and the wider community for prompt diagnosis, treatment, and survivorship.


Evaluation:
Monitors patient progress, goal achievement, and feedback to assess care effectiveness. Communicates any changes in the care plan to patients, families, and the healthcare team. Maintains an understanding of the healthcare team's treatment strategies and professional guidelines.

Engages in quality improvement activities that support optimal patient care and outcomes, aligning with patient and family preferences.

Demonstrates the necessary competencies, including knowledge and behaviors, to fulfill specific patient care needs, nursing standards, and regulatory requirements. Keeps abreast of clinical improvement strategies and outcome management through ongoing education and professional development.

Promotes a professional atmosphere that encourages shared decision-making, interdisciplinary collaboration, and high levels of patient and employee satisfaction.
