As a Care Coordinator for our Behavioral Health Case Management team, you will play a vital role in improving the lives of individuals struggling with mental health issues.

About the Role

This is an ongoing nursing job requiring 36 hours per week, with a 12-hour shift schedule. You will work closely with highly trained professionals to provide support and guidance to individuals in need. The ideal candidate will have a strong understanding of behavioral health principles and experience working with vulnerable populations.

Key Responsibilities
  • Provide emotional support and guidance to individuals in crisis
  • Develop and implement personalized care plans to meet individual needs
  • Collaborate with healthcare teams to ensure seamless care transitions
  • Conduct thorough assessments to identify risk factors and develop targeted interventions
Requirements

To be successful in this role, you must possess a High School Diploma and at least one year of related work experience. You should also be certified as a Peer Support Specialist or enrolled in training to obtain certification within six months of hire.

  • Certified Peer Support Specialist designation preferred
