Job Overview:

Lutheran Social Services of Central Ohio is seeking a highly motivated and compassionate Community Outreach Coordinator to join our team in Fairfield County. As a Community Outreach Coordinator, you will play a vital role in serving individuals experiencing homelessness and those struggling with addiction.

About the Role:

This is a full-time position working day shifts from 8am-4pm, Monday through Friday. You will be responsible for conducting street outreach in the community and on-site intake assessments in partnership with the Outreach Specialist. Your primary goal will be to link clients to essential services, including housing, employment, medical, and mental health resources.

Key Responsibilities:

  • Conduct VI-SPDAT assessments for housing and Coordinated Entry (CE) assessments to identify client needs
  • Link clients to services such as AOD treatment, harm reduction, resources, supports, mental health, and medical services
  • Assist clients in obtaining necessary documentation for housing, employment, and other benefits
  • Establish and maintain empathetic relationships with clients, providing assertive outreach when needed
  • Build partnerships with community stakeholders and agencies to enhance service delivery
  • Pursue ongoing professional growth and development through required trainings

Requirements:

  • High School Diploma or equivalent required; Certified Peer Support Specialist certification preferred
  • Prior experience working with homeless populations and/or AOD/Mental Health services desired
  • Valid Ohio driver's license, reliable insured transportation, and ability to meet LSS insurance standards required
