- Referral Process: Facilitate referrals to Advocate Aurora Health departments as needed.
- Documentation: Maintain accurate and up-to-date records of patient interactions and care coordination activities.
- Licensed Practical Nurse (LPN) license issued by the state in which the team member practices.
- A valid driver's license issued by the Division of Motor Vehicles.
- Completion of an accredited or approved program in Practical Nursing.
- Typically requires 3 years of experience in a LPN role.
- Excellent telephone, interpersonal, and written communication skills.
- Knowledge of Medicare rules and regulations.
- Strong commitment to teamwork and customer service.
This position requires the ability to sit, stand, walk, lift, squat, bend, reach above shoulders, and twist frequently throughout the workday. Must be able to lift up to 50 lbs. from floor to waist, lift up to 20 lbs. over the head, and carry up to 40 lbs. a reasonable distance.
