NHTD/TBI Waiver Program Coordinator

We are looking for a dedicated NHTD/TBI Waiver Program Coordinator to support prospective and active participant and family support with frequent communication and follow up, verification of eligibility for assistive programs and maintenance of accurate and comprehensive documentation.

The successful candidate will provide step-by-step assistance throughout the Waiver program approval process, offer and facilitate access to other forms of support throughout the NHTD/TBI Waiver program approval process, including other NYS public benefits and community resources.

This role requires a bachelor's degree in social work or higher, two years of experience providing case management/service coordination, information and referral services and brokering of community-based services for individuals with disabilities and/or Traumatic Brain Injury or seniors who are medically fragile.

The estimated salary for this position is $70,000 - $80,000 per year, depending on experience.
