About the Role

Aveanna Healthcare is seeking a dedicated and compassionate Private Duty Nurse LVN to join our team. As a Private Duty Nurse, you will play a vital role in delivering high-quality, personalized care to our patients in the comfort of their own homes.

Key Responsibilities
  • Implement physicians' prescribed plans of care to ensure optimal patient outcomes
  • Conduct patient assessments and coordinate care with our multidisciplinary team
  • Administer prescribed medication, treatments, and therapies as ordered by physicians
  • Document care provided in our electronic system of record, ensuring accurate and timely reporting
  • Provide health education and training to patients and their families, promoting healthy habits and disease management
  • Collaborate with our Case Manager to ensure seamless care transitions and address patient concerns
Requirements
  • Active, unencumbered license (RN/LPN/LVN) in the state of practice
  • Current CPR certification from American Red Cross, American Heart Association, or BLS
  • TB skin test (current within last 12 months)
  • At least six months of prior hands-on nursing experience (preferred but not required)
  • Reliable transportation
