Case Management RN Job Description

We are seeking a skilled and compassionate Registered Nurse to join our Case Management team at TriStar StoneCrest Medical Center.

About the Role:

  • Provide case management services for inpatient and observation patients, identifying those at risk for adverse outcomes during transitions of care.
  • Conduct comprehensive assessments of patients' psychosocial, medical, and discharge needs, as well as their access to resources.
  • Develop strategies to mitigate readmission risk, including education on accessing healthcare resources and community-based services.
  • Collaborate with the multidisciplinary healthcare team to drive the discharge plan and facilitate successful care transitions.
  • Partner with Social Services to ensure post-acute medical needs and level of care are appropriate.
  • Evaluate progression of care using evidence-based tools and approved criteria, escalating issues through the established chain of command.
  • Make referrals to third-party payers and disease and case management programs for recurring patients and those with chronic disease states.
  • Facilitate patient throughput with a focus on effective care transitions, quality, and efficiency.
  • Document professional recommendations, discharge plans, care coordination interventions, and case management activities.
  • Act as a liaison between physicians, patients, hospital staff, and outside agencies, ensuring effective communication.
  • Seek ways to control costs without compromising patient safety, quality of care, or services delivered.
  • Direct activities to identify and provide for the needs of under-resourced patient populations, including patient education and community-based resources.
  • Participate in performance improvement activities, including identifying and intervening in avoidable days.
  • Adhere to established policy and procedure, demonstrating knowledge of regulatory requirements and quality initiatives.
  • Ensure patients' ethnic, cultural, or religious values, beliefs, preferences, and needs are considered and aligned.

Requirements:

  • Associate Degree in Nursing or Nursing Diploma
  • 2+ years of experience in case management or 3+ years of experience in clinical nursing
