The Homeless Services Advocate plays a crucial role in providing ongoing case management, ensuring that clients receive the necessary resources to achieve permanent housing and stability.
Key Responsibilities:
- Respond to crisis hotline inquiries, offering immediate support to those experiencing homelessness or at risk.
- Evaluate eligibility for various programs and services.
- Collect and complete necessary documentation for financial assistance requests.
- Maintain accurate records in the Homeless Management Information System (HMIS) and agency databases.
- Deliver emergency counseling and crisis intervention services.
- Adhere to all relevant Federal, State, and Local regulations.
- Conduct assessments to identify client strengths, needs, and goals, and create actionable plans.
- Monitor client progress and adjust plans as needed.
- Conduct on-site visits to provide direct support.
- Facilitate connections to community resources and services.
- Act as a mediator between clients and landlords or service providers when necessary.
- Assist clients in obtaining essential supplies and services.
- Ensure timely and accurate documentation of all interactions and services provided.
- Educate clients on preventing future housing crises.
- Coordinate transportation for clients to access necessary services.
- Collaborate effectively with other agencies and community partners.
- Provide guidance on financial management and home maintenance.
- Engage in additional duties and special projects as assigned.
- Participate in training and professional development opportunities.
Qualifications:
- Ability to connect with diverse populations facing various challenges.
- Demonstrated ability to engage with clients in a compassionate and supportive manner.
- Strong relationship-building skills with service providers and colleagues.
- Self-motivated, organized, and adaptable to changing needs.
- Flexibility in work hours to accommodate program requirements.
- Commitment to a non-judgmental, trauma-informed approach.
- Excellent verbal and written communication skills.
- Professional demeanor with sound judgment and resourcefulness.
- Valid New York State Driver's License and reliable transportation for client travel.
Education and Experience:
- Associate's Degree in Behavioral Sciences or Human Services, and
- One to three years of relevant experience in human services or housing programs, or
- A combination of education, experience, and training that meets the requirements.
Work Schedule: 37.5 hours per week, Monday through Friday, with required flexibility.
