Job Overview

FUNCTIONAL DEFINITION
This role encompasses a comprehensive suite of case management services tailored for individuals facing challenges related to substance use. The professional in this position ensures that clients are seamlessly connected to a spectrum of care by identifying, referring, and linking them to suitable resources. Key responsibilities include evaluating client needs, establishing appropriate care levels, crafting service plans, and facilitating treatment interventions aimed at addressing substance use issues when necessary. Building a trusting relationship with clients is crucial, as this individual acts as their advocate, guiding them through their service plan and monitoring their progress.

KEY RESPONSIBILITIES

  1. Conduct thorough assessments through interviews to gather essential information from clients, families, and relevant parties to identify necessary services.
    1. Assess and determine the appropriate level of care based on established placement criteria.
  2. Identify suitable funding options for clients dealing with substance use challenges.
  3. Guide clients through the treatment journey:
    1. Serve as the primary resource for the client
    2. Revise the client’s service plan as required
    3. Coordinate access to available support services, including housing, childcare, medical, and financial assistance
    4. Prepare for and participate in treatment planning meetings, providing follow-up as needed
    5. Monitor the client’s progress throughout treatment
  4. Maintain accurate client records and prepare detailed reports.
  5. Manage a caseload effectively under the supervision of higher-level administrative staff.
  6. Oversee the delivery of services.
  7. Act as an advocate for clients.
  8. Perform additional duties as assigned.

SKILLS AND QUALIFICATIONS

  1. Preferred experience and training in social work principles and methodologies.
  2. Understanding of addiction as a medical condition and its treatment modalities.
  3. Awareness of contemporary social, economic, and health issues along with available resources.
  4. Capability to connect clients with appropriate treatment facilities and human services organizations.
  5. Ability to interpret and apply regulations, policies, and procedures effectively.
  6. Experience working collaboratively with diverse agencies and communities.
  7. Competence in assisting individuals with various physical, emotional, or mental health challenges.
  8. Strong communication skills.
  9. Ability to thrive in a team-oriented environment.
  10. Effective organizational skills for managing records, reports, and caseloads in a timely manner.

ESSENTIAL REQUIREMENTS

  1. Possession of required clearances.
  2. Valid driver’s license and access to a personal vehicle with appropriate insurance coverage.
  3. Ability to work autonomously.
  4. This is a full-time position requiring 38.75 hours per week.
  5. Willingness to pursue relevant certifications.

EDUCATION AND EXPERIENCE

A Bachelor’s Degree in Chemical Dependency, Sociology, Social Work, Psychology, Nursing, or a related discipline, along with one year of direct service experience in counseling or case management within a substance use treatment context. Alternatively, any equivalent combination of experience and training.
