Job Title: Addiction Case Manager

Job Summary:

The Addiction Case Manager will provide comprehensive case management services to clients diagnosed with drug or alcohol problems. This role ensures that clients have access to a continuum of care by identifying, referring, and connecting them to appropriate services.

Key Responsibilities:

  • Assessment and Planning: Conduct interviews to assess client needs, determine the level of care, and develop a service plan.
  • Service Coordination: Coordinate and provide access to available alcohol and other drug services, as well as ancillary services such as housing, childcare, medical, or financial services.
  • Treatment Planning and Progress Monitoring: Prepare for, participate in, and provide follow-up after treatment planning meetings, and monitor client treatment progress.
  • Documentation and Reporting: Prepare written reports and maintain client records.
  • Caseload Management: Manage client caseload and carry out work under the direction of a Supervisor or higher level administrative personnel.
  • Advocacy: Serve as an advocate for clients.

Requirements:

  • Education: Bachelor's Degree in Chemical Dependency, Sociology, Social Welfare, Psychology, Nursing, or a related field.
  • Experience: One-year direct service experience performing counseling or case management work in an alcohol or other drug treatment setting.
  • Certifications: Possess Act 33 and 34 Clearances, and have a valid Pennsylvania motor vehicle license, access to a personal vehicle, and maximum auto insurance on an employee-driven vehicle.

Working Environment:

This is a full-time position requiring 38.75 hours per week. The ideal candidate will be willing to pursue a CAAP credential or other similar state certification.
