Care Management Specialist

As a Care Management Specialist at Vynca, you will play a pivotal role in transforming care for individuals with complex needs. Your exceptional leadership skills and experience in care management will enable you to oversee client cases, coordinate health care benefits, and provide education to facilitate member access to care in a timely and cost-effective manner.
The ideal candidate will have 2+ years of experience as a care manager, care navigator, or community health worker supporting vulnerable populations. A working knowledge of government and community resources related to social determinants of health is also essential. Excellent oral and written communication skills, positive interpersonal skills, and general computer skills are required.
We offer a competitive salary range of $26-$30/hour, mileage reimbursement per IRS guidelines, and great benefits, including medical, dental, and vision insurance, income protection benefits, PTO, company holidays, a 401k, and access to other wellness benefits.
Key Responsibilities:
- Assess member needs in areas of physical health, mental health, SUD, oral health, palliative care, memory care, trauma-informed care, social supports, housing, and referral to community-based services and supports
- Oversee the development of client care plans and goal settings
- Offer services where the member resides, seeks care, or finds most easily accessible
- Connect clients to other social services and supports needed
- Advocate on behalf of the client with healthcare professionals
- Utilize evidence-based practices, such as Motivational Interviewing, Harm Reduction, and Trauma-Informed Care principles
