Job Summary

Therapeutic Alternatives is seeking a highly skilled and compassionate Hospital Transition Team Qualified Professional to join our team. As a key member of our Community Support Team, you will play a vital role in providing short-term services to individuals transitioning from psychiatric hospitals or residential substance abuse treatment programs.

Key Responsibilities
  • Provide crisis response services 24/7/365, ensuring the safety and well-being of our clients.
  • Develop and implement treatment plans tailored to each client's unique needs and goals.
  • Collaborate with community resources to provide one-on-one intervention and support, focusing on interpersonal and coping skills development.
  • Monitor client symptoms, medications, and self-management strategies, providing therapeutic mentoring and guidance as needed.
  • Coordinate case management services, linking clients with multiple resources and assessing their ongoing needs.
  • Inform clients about available benefits and services, arranging for provision and monitoring their delivery.
  • Work closely with providers to ensure seamless service delivery and advocate on behalf of clients.
  • Coordinate movement across levels of care, facilitating discharge planning and community re-entry.
  • Complete required training and documentation, including crisis management and Community Support Team activities.
Requirements
  • Minimum one year of experience in crisis management services, preferably in assertive outreach, assertive community treatment, or emergency department settings.
  • Valid NC Driver's License and at least 8 years of driving experience.
  • QP licensure, certification, registration, or permit issued by a governing board regulating a human service profession.
  • Master's or Bachelor's degree in a human service area, with relevant experience working with individuals within disciplines.
