About the Role:
As a Hospice Registered Nurse - Per Diem at Talbot Hospice, you will be responsible for coordinating and managing patient care plans, ensuring the highest standards of comfort and support during this sensitive time. You will work closely with interdisciplinary teams to deliver exceptional care and support to patients and their families. Your role will be integral to upholding our company's mission of delivering dignified and compassionate end-of-life care.
Key Responsibilities:
- Develop and implement individualized care plans that address the unique needs of each patient.
- Collaborate with interdisciplinary teams to ensure seamless care coordination and delivery.
- Provide skilled nursing care, including symptom management and pain control, to patients and their families.
- Educate patients and families on the hospice philosophy and care options.
- Maintain accurate and timely documentation in accordance with regulatory standards.
Requirements:
- Graduate of an accredited school of nursing with one to two years of recent acute care experience in an institutional setting and one to two years of recent experience in hospice or home care.
- Current Registered Nurse RN Licensure in State of Maryland and CPR certification.
- Excellent observation, verbal and written communication skills, problem-solving skills, basic math skills, and nursing skills per competency checklist.
- Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
- Per diem RN's are required to work 2 weekend shifts within a 6-week period.
Estimated Salary: $31 to $50 per hour.
