Job Title: RN - Hospice - Weekend On Call

PruittHealth Hospice is seeking a compassionate and skilled Registered Nurse to join our team as a Weekend On Call RN. As a member of our interdisciplinary care team, you will provide high-quality care to patients and their families, ensuring a seamless transition from hospital to home.

Job Summary:

The RN - Hospice - Weekend On Call will be responsible for:

  • Screening referrals for appropriateness and performing admissions to facilitate the transition of patients and families into the hospice program.
  • Case managing patients and families to ensure symptom management, patient and family education, medication management, coordination of hospice services, and excellent communication with the patient's physician.
  • Providing primary nursing care for hospice patients, regardless of the setting.
  • Assuming full responsibility for professional management of assigned hospice patients' care related to the patient's terminal illness.
  • Teaching the primary caregiver skills needed to care for the patient in their residence.
  • Working closely with the attending physician and other team members to develop appropriate pain and symptom management.
  • Completing nursing assessments and admissions of patients.
  • Maintaining the clinical record to meet federal, state, and agency regulations and guidelines.
  • Supervising hospice aides and certified nurse aides, making supervisory visits at least every 14 days, and face-to-face at least monthly.
  • Assuming all on-call responsibilities as assigned.
  • Participating in Performance Improvement Program.
  • Participating in and presenting inservice programs, team conferences, and staff meetings.
  • Providing skilled nursing care to assigned patients/families as outlined in the plan of care.
  • Participating in the development of the patient/family interdisciplinary care plan.
Requirements:

To be successful in this role, you will need:

  • A current, active, and unrestricted Registered Nurse (RN) licensure in the state of practice.
  • Excellent communication skills, both oral and written.
  • Excellent organizational skills.
  • Ability to handle confidential matters.
  • Ability to work a flexible schedule.
  • Commitment to the Hospice concept and standards.
  • Ability to interact with a variety of different age groups.
  • Participation in agency surveys and any subsequently required reports.
  • Attendance and participation in continuing educational programs to keep abreast of changes in your field as well as to maintain current license/certification, as required.
  • Attendance and participation in mandatory in-services.
  • Honor patients/residents' rights to fair and equitable treatment, self-determination, individuality, privacy, property, and civil rights, including the right to wage complaints.
  • Compliance with corporate compliance program.
  • Reporting job-related functions/tasks that involve occupational hazards, including exposure to blood and body fluids, and others as necessary.
  • Following established safety regulations, to include fire protection & prevention, smoking regulations, infection control, etc.
  • Following established safety procedures when performing tasks and/or working with equipment.
