Job Title: Case Manager

At Women In Need, Inc., we are seeking a highly skilled and compassionate Case Manager to join our team. As a Case Manager, you will play a vital role in providing case management services to clients in transitional housing, ensuring their successful integration into both housing and employment goals.

Key Responsibilities:
  • Conduct initial assessments of clients within 48 hours of arrival and complete necessary documentation.
  • Initiate referrals for emergency services and ensure timely follow-up.
  • Complete intake information and ensure accurate data entry in the Uniform Client Chart.
  • Conduct client unit inspections and review findings with clients and supervisors.
  • Provide comprehensive case management, including crisis intervention, counseling services, and support services.
  • Develop Independent Living Plans (ILPs) for clients, tracking progress and making modifications as needed.
  • Assist clients in obtaining income and public benefits, as well as establishing connections with medical and mental health services, education/job training programs, and other community resources.
Requirements:
  • Bachelor's Degree in social work or related field preferred, with two years of relevant experience or an Associate's Degree and three years of client-facing experience.
  • Familiarity with Evidence-Based Practices (EBPs) and entitlement systems preferred.
  • Excellent organizational, written, and verbal communication skills, with ability to work effectively in a team environment.
  • Computer skills and knowledge of CARES a plus, with bilingual English/Spanish proficiency also a plus.
