Licensed Practical Nurse

Community Access Services is seeking a skilled Licensed Practical Nurse (LPN) to join our team. As a LPN, you will provide vital healthcare services to individuals with intellectual and developmental disabilities (IDD) residing in a group home or residential setting.
Key Responsibilities:- Provide skilled nursing care, including medication administration, hygiene assistance, and mobility support.
- Collaborate with the RN manager and group home manager to provide emergency caregiving services as needed.
- Assess daily needs by reviewing medical progress notes and other documentation.
- Complete accurate and timely documentation of care provided, and any incidents or observations.
- Active LPN license in good standing in the state of Oregon
- Proficient knowledge in tube feeding, care of J & G tubes, nebulizer treatments, IV maintenance, and familiarity with various pumps.
- Competency in airway maintenance, trach care, ventilation, and suctioning.
- Strong understanding of medication administration protocols and awareness of potential side effects.
- Current CPR/first aid certification.
- Ability to acquire Basic Life Support (BLS) certification and IV certification within 3 months of hire.
- Must be able to pass a level II background check
- Capable of performing physical tasks, including lifting up to 50 lbs.
