As a key member of our admissions team, you will be responsible for conducting pre-admission evaluations, educating patients and families about our services, and ensuring a smooth transition into our care.

Key Responsibilities
  • Conduct thorough pre-admission evaluations to assess the appropriateness of our services for patients and families.
  • Provide education and support to patients and families about our hospice care services, including benefits, eligibility, and the admission process.
  • Obtain informed consent from patients and families, ensuring they understand the services we provide and their rights as patients.
  • Develop and implement individualized plans of care, working collaboratively with our interdisciplinary team to optimize patient comfort, quality of life, and family support.
Requirements
  • Graduate from an accredited registered nursing program.
  • Licensed as a registered nurse in the state of practice.
  • Current CPR certification.
  • 1 year of hospice experience preferred.
  • Ability to make independent decisions and work effectively in a fast-paced environment.
  • Excellent communication and interpersonal skills, with the ability to work with patients, families, and healthcare professionals.
