Job Title: Supervisor, Community Outreach

Upward Health is a pioneering healthcare organization dedicated to delivering innovative, patient-centered care to underserved communities. We are seeking a highly motivated and experienced Supervisor, Community Outreach to join our team.

About the Role

The Supervisor, Community Outreach will play a critical role in building and maintaining relationships with healthcare providers, community-based organizations, and social service agencies. This position will be responsible for supervising a team of Community Outreach Specialists, ensuring the successful execution of outreach activities, and fostering a culture of excellence within the team.

Key Responsibilities
  • Supervise and mentor a team of Community Outreach Specialists to ensure the delivery of high-quality outreach services.
  • Develop and implement effective outreach strategies to engage patients, healthcare providers, and community-based organizations.
  • Collaborate with the Clinical Services team to ensure seamless patient handoffs and continuity of care.
  • Conduct regular team meetings and provide feedback to ensure the team is meeting its goals and objectives.
  • Develop and maintain relationships with healthcare providers, community-based organizations, and social service agencies to promote Upward Health's services.
  • Identify and address any operational or logistical challenges that may impact the team's performance.
  • Prepare and submit regular reports to management on team performance, outreach activities, and patient engagement metrics.
Requirements
  • Minimum of 5 years of experience in a healthcare-related field, with a focus on patient care delivery, public health initiatives, or population health.
  • Minimum of 2 years of supervisory experience, with a proven track record of leading high-performing teams.
  • High school graduate or equivalent required; associate's or bachelor's degree in a healthcare-related field preferred.
  • Valid driver's license and reliable transportation required.
  • Strong communication and interpersonal skills, with the ability to build and maintain relationships with diverse stakeholders.
  • Ability to work independently and as part of a team, with a strong focus on collaboration and teamwork.
  • Strong problem-solving and analytical skills, with the ability to identify and address operational challenges.
Preferred Qualifications
  • Associates or bachelor's degree in a healthcare-related field, with a focus on public health, health education, or a related field.
  • Multi-lingual capabilities a plus, but not required.
  • Strong knowledge of community resources, social services, and healthcare systems.
  • Experience working in a start-up or entrepreneurial environment, with a focus on innovation and adaptability.
