Key Responsibilities- Assess and assist with managing patients' total needs following physician orders with RN oversight and utilization of nursing process
- Responsible for nurse visits and triage schedule, including assessment and patient evaluation related to lab values, administering medications and treatments in accordance with the patient's plan of care
- Perform routine nursing procedures to meet the needs of patients under the supervision of RN, including accessing and de-accessing ports, flushing lines, and sterile technique
- Manage prior authorization for certain medications, excluding chemotherapy medications, and assist patients with filling out paperwork regarding leave of absence, free drug programs, and financial assistance programs as needed
- Coordinate care and services with RN and other team members, including informing RN, physician, midlevel, and/or clinical supervisor or manager of changes in patients' condition, as appropriate
- Graduate from a practical or vocational nursing program
- Use of Windows-based computer, utilizing home health patient software, email, internet, computer-based learning modules, etc.
