Job Summary:

Responsibilities:

  • Conduct thorough evaluations, assessments, and treatments in accordance with policy and individualized care plans.
  • Maintain accurate and timely documentation and records.
  • Follow established policies and procedures related to the scope of practice.
  • Ensure continuity of patient care in a home care setting.
