The Hutchinson Regional Medical Center, Inc. is seeking a highly skilled and compassionate Registered Nurse to fill the role of On-Call/Afters Hours Registered Nurse. This position requires a strong commitment to providing exceptional patient care and support, as well as excellent communication and problem-solving skills.
Key Responsibilities:- Triage and Assessment: Triage calls from patients and caregivers, ensuring an accurate evaluation of patient needs and developing a comprehensive care plan that addresses those needs.
- Home Visits: Make timely home visits to evaluate and provide care as directed on the individualized plan of care to achieve desired patient outcomes.
- Collaboration and Communication: Collaborate and maintain communication with physician, patient/family, RN Case Manager, and other assigned disciplines regarding care needs and revisions to plan of care.
- Documentation and Reporting: Communicate response to triage and patient care information in a timely manner to patient care team, leadership, etc.
- Pain Management and Symptom Control: Demonstrate knowledge of pain management and symptom control, educate patient/family on physical, emotional and psychosocial aspects of care, disease process, symptom and pain management, and signs and symptoms of death.
- Medication Administration: Provide for accurate administration and evaluation of medication, management of medication orders, and delivery of medication to patient in a timely manner.
- Admission Assessments: Complete hospice admission assessments, home health admission assessment (OASIS) and develops patient's initial plan of care.
- Supervision of Services: Complete supervision of HHA services provided as per policy.
- On-Call Responsibilities: Participate in on-call responsibilities as needed to ensure 24 hour nursing availability for agency hospice, homecare and private duty patients.
- Documentation and Reporting: Complete timely and accurate documentation as per agency policy.
- Relationship Building: Establish and maintain positive working relationships with patients, family members, co-workers, physicians and referral sources.
- Patient Education: Promote patient's independence by establishing patient care goals and including the patient and family in discussions regarding medical issues, testing, medications, and self-care skills.
- Observation and Collaboration:
- Observe the patients' physical, mental, emotional, and spiritual conditions and collaborate with team members as appropriate.
- Education: Graduate of an accredited school of nursing.
- Experience: Several years of clinical experience.
- Licensure: Current licensure as a RN with the State of Kansas.
- Certifications: BLS - BLS is required within three months of hire.
- Driver's License: Kansas State Driver's License.
- Insurance: Auto Insurance equal to or greater than the Kansas insurance requirements.
- Education: BSN.
- Certifications: Certified as Hospice and Palliative Care Nurse.
- Experience: Previous hospice and/or home health experience.
